How to Use MyChart

Use MyChart to view your medical records, message your doctor, and manage appointments online.

Steps

Go to mychart.com or download the MyChart app
from the App Store or Google Play. Search for your hospital or clinic.

Sign up for an account
by entering your name, date of birth, and the activation code from your doctor's office. You can request one at your next visit.

Log in with your username and password
that you create during sign up. You can use your fingerprint or face to log in on your phone.

Explore the main menu
to view test results, send messages to your doctor, request prescription refills, and see upcoming appointments.
